Scope of this policy
This policy covers the public ICRS website and its consultation-request form. It does not replace the practice's clinical Notice of Privacy Practices or govern information collected during care. Do not use this website for emergencies, detailed medical information, photographs, attachments, payment-card information, or time-sensitive concerns.
Information the public form accepts
The consultation form is intentionally limited to your name, phone number, email address, procedure interest, general timeframe, preferred contact method, and the consent choices you make. The form does not provide a free-text box or accept uploads. We use this information to respond to your request, coordinate a consultation, operate and secure the website, and meet recordkeeping obligations.
How information is handled
ICRS does not sell personal information or use advertising pixels, session replay, or invasive behavioral tracking on this website. Access is limited to people and service providers who need it to operate the consultation process, protect the service, or comply with law. We do not share mobile numbers or text-message opt-in data with third parties or affiliates for their own marketing purposes.
Optional text messages
Text-message consent is separate from a consultation request and is never preselected. If you choose it, ICRS may send neutral messages about consultation scheduling, forms, and account updates. Consent is not a condition of receiving services. Message frequency varies; message and data rates may apply. Reply STOP to opt out or HELP for help. Retention, security, and choices
We retain consultation-request information only as needed for the request, operations, security, and applicable recordkeeping requirements. Reasonable safeguards are used to protect information, but no internet transmission can be guaranteed completely secure. You may ask about or update the contact information you gave us, or withdraw optional text-message consent, by using the contact details below. Withdrawing text-message consent does not withdraw a request to be contacted by another method.
